NICHQ's 4th Annual Forum for Improving Children's Health Care

Innovation Communities
Learning and Innovation Community: Reducing Hospital-Acquired Infections (MRSA, VRE, C. diff)
 FOR CURRENT MEMBERS ONLY We are no longer accepting new participants in this Community. Health care-associated infections remain a major cause of morbidity, mortality and cost despite concerted efforts of infection control professionals for nearly a half-century. Recently, treatment of these infections has become more complex due to an alarming rise in antibiotic resistance. The aim of this Community will be to significantly reduce health care-associated infections due to MRSA, VRE, and C. diff.
Learning and Innovation Community: Reducing Surgical Complications
 FOR CURRENT MEMBERS ONLY We are no longer accepting new participants in this Community. A substantial percentage of operative procedures result in preventable complications, some of which can be life-threatening. The aim of this Learning and Innovation Community is to significantly reduce the occurrence of the most serious surgical complications. Specific areas of focus include infections, cardiac events, emboli, and post-operative bleeding.
Learning and Innovation Community: Reducing Hospital Mortality Rates
 Organizations have the option to participate in this Community either through membership in IMPACT or through direct enrollment in the Community. The Reducing Hospital Mortality Rates community will focus on decreasing mortality rates by addressing the system issues that contribute to mortality, and implementing changes for which strong evidence already exists.
Learning and Innovation Community: Implementing an Idealized Model for Critical Care
 Organizations have the option to participate in this Community either through membership in IMPACT or through direct enrollment in the Community. Despite numerous local improvements in various elements of critical care, hospitals have barely tapped the potential of these improvements or the potential of emerging technologies. The Critical Care community is designed to close the gap between what is known and what is practiced, and to establish new systems of care in critical care settings.
Innovation Community: Designing for Reliability in Clinical Processes
For Current Members Only  We are no longer accepting new participants in this Community. The Designing for Reliability community will focus on dramatically increasing reliability in clinical diagnoses and processes, for which clinical evidence already exists and has been recommended by expert groups or adopted by national organizations.
Innovation Community: Improving Access and Efficiency in Specialty Practices
Begins Fall 2004  Participants will test new ideas in an effort to achieve measurable improvements in specialty practice access and efficiency.
Innovation Community: Building an Effective "Planned Care" System for All Patients in Ambulatory Settings
Begins Fall 2004  This Innovation Community will take what is known about chronic disease management and resource planning in office practices and test new methods that create measurable improvements in chronic care across conditions.
Innovation Community: Reducing Mortality Rates (HSMR)
Begins Fall 2004  Participants will take steps to reduce overall HSMR by 20 points or by a 25% reduction in raw mortality.
Innovation Community: Improving Flow Through Acute Care Settings
Begins October 2004  Participants will be asked to design and test changes aimed at increasing overall hospital throughput.
BTS Collaborative: Reducing Surgical Site Infections — Deploying a High-Reliability System
Begins October 2004  Participants in this Breakthrough Series (BTS) Collaborative will decrease morbidity and mortality associated with postoperative infection through various measures, including: appropriate and timely prophylactic antibiotic administration; perioperative glucose control and supplemental oxygen; controlling perioperative normothermia; and improving operating room culture.
Innovation Community: Improving Reliability in Key Clinical Areas
Begins October 2004  The project will focus on dramatically increasing reliability in five clinical diagnoses: acute myocardial infarction (AMI); coronary artery bypass graft (CABG); heart failure; community-acquired pneumonia; and hip and knee replacement.
BTS Collaborative: Improving Access and Efficiency in Primary Care
Begins October 2004  Participating organizations in this Breakthrough Series (BTS) Collaborative will apply three strategies for building an access system that offers patients appointments with their primary care provider on the same day they call: shape demand, match supply and demand, and redesign their office practice systems to increase capacity.
BTS Collaborative: Reducing Complications from Ventilators and Central Lines in the ICU
Begins September 2004  The goals of this Breakthrough Series (BTS)Collaborative will be to significantly reduce ventilator-associated pneumonia and catheter-related bloodstream infections through the use of ventilator bundles, central line bundles, multidisciplinary rounds, daily goal sheets, and effective weaning protocols.
BTS Collaborative: Reducing High-Hazard Adverse Drug Events — Narcotics and Anticoagulants
Begins September 2004  This Breakthrough Series (BTS) Collaborative will focus specifically on reducing adverse drug events (ADEs) in narcotics and anticoagulants, specifically: measuring harm, reconciliation of medications, building a safety culture, and improving core medication processes.
