Definition
The number of patients with a self-management goal negotiated or renegotiated at last asthma visit divided by the number of patients in the population of focus. Multiply by 100 to get percent.
Goal
To have greater than 70 percent of patients with a self-management goal.
Data Collection Plan
On the last workday of each month, from flow sheet/chart information or registry:
- Count the number of patients with a working self-management goal at last asthma visit.
- Count the total number of patients. Exclude patients who have not been entered into the registry.
Flow sheets and the registry will need a field to indicate that a self-management goal has been negotiated or renegotiated (= "working").
