Based on error reports submitted to the Institute for Safe Medication Practices (ISMP) Medication Errors Reporting Program, reports of harmful errors in the literature, and input from practitioners and safety experts, ISMP created and periodically updates a list of potential high-alert medications.

These tools were developed through a collaborative effort of the American Hospital Association, the Health Research and Educational Trust, and the Institute for Safe Medication Practices. [USA]

The American Society of Health-System Pharmacists (ASHP) is the 30,000-member professional association that represents pharmacists who practice in hospitals and other components of health care systems. A long-time champion of medication-use safety, ASHP believes that the mission of pharmacists is to help people make the best use of medicines. Toward that end, the Society has extensive publishing and educational programs designed to help health-system pharmacists improve their delivery of pharmaceutical care, and it is the national accrediting organization for pharmacy residency and technician training programs. The Institute for Safe Medication Practices (ISMP) is a nonprofit agency that provides information about adverse drug events and their prevention to healthcare practitioners and institutions, consumers, regulatory agencies, professional organizations and the health care industry at large. In cooperation with the United States Pharmacopeia, ISMP operates the national Medication Errors Reporting Program (USP-ISMP MERP). Their website includes research findings, books and publications, an electronic discussion forum, and links to related sites. Featured are back issues of the ISMP Medication Safety Alert! which chronicles medication error reports received from the USP-ISMP MERP and the US Food and Drug Administration's Medication Errors Reporting Program and provides analysis and expert recommendations for prevention. State Initiatives in the USA

The Massachusetts Coalition for the Prevention of Medical Errors was established to improve patient safety and minimize medical errors. The goals of the coalition are to: Establish a mechanism to identify and implement best practices to minimize medical errors; Increase awareness of error prevention strategies through public and professional education; and Identify areas of mutual interest and minimize duplication of regulatory and Joint Commission on Accreditation of Healthcare Organizations (JCAHO) requirements so that efforts are focused on initiatives that can best improve patient care.
