This tool is to be used during shift changes and handoffs to ensure seamless patient care. The tool was designed to improve handoff communication among staff using the SBAR Technique for Communication (Situation-Awareness-Background-Recommendation).

The tool was initially piloted with radiology and nursing staff. After many updates and iterations, this document has become extremely useful to staff, and the hospital is now spreading the document to other departments.
