This tip sheet provides surgical patients with key questions they should ask their surgical care team prior to surgery. The tool is intended to be used as a communication tool between surgical patients and caregivers to help reduce potential risks.

The Surgical Care Improvement Project (SCIP) is a national quality partnership of organizations focused on reducing the incidence of surgical complications nationally by 25 percent by the year 2010. SCIP Partners include a Steering Committee of 10 national organizations, including the Institute for Healthcare Improvement; Supporting Partners; and members of Technical Expert Panels.
